SB 12 – Parental Rights, Instructional Plans, and Consent Forms - Routine Services and School Clubs/Organizations

Senate Bill 12 requires school districts to provide the parental rights form to parents upon their child’s enrollment in the district for the first time and to the parent of each child enrolled in the district at the beginning of each school year. It further requires each district to post the form in a prominent location on the district’s website.

Parental Rights and Options Form

SB 12 requires school districts to make available on the district’s website an instructional plan for each class offered. The goal is to keep parents informed of the curriculum in which their students will be learning by grade level and content. CMISD maintains that it will ensure parents are informed of the instruction being provided to their students and will provide additional information, as necessary, if parents request it. Caddo Mills ISD is a District of Innovation, and the CMISD District of Innovation Plan may be found here.

SB 12 requires schools to get annual parental consent before providing routine services, including health services, physical or mental health services. This includes services such as first aid, short-term counseling, or vision and hearing screenings. SB12 also requires written parental consent for any club or organization the district authorizes or sponsors. The student clubs/organizations form is distributed to students by the club/organization sponsor. You can also access the student clubs/organizations form for Caddo Mills High School here and Caddo Mills Middle School here.

SB 12 requires an internet portal through which parents of students enrolled in the district may submit comments to campus or district administrators and the board.  The CMISD Parent Engagement Portal can be found here.

To complete the parental consent forms, log into your Skyward Family Access account and locate the 2025-2026 Parental Consent Form tile. Review and indicate whether you consent or do not consent to:

  • Health services

SB 13 - Parent/Guardian Access to Child’s School Library Activity

SB 13 requires procedures for parents to access the catalog of available library materials at each school library, for parents to be allowed to submit a list of materials their child may not be allowed to check out or otherwise have access for use outside of the library, and the district’s online portal must provide parents a record of each time their child checks out library materials.

CMISD parents/guardians now have access to the CMISD Library Guardian Portal to help them stay informed about their child’s school library activity. They can:

  • Find summaries of all books in the school library

  • Search books by reading level and subject

  • Create book lists to manage their child’s reading

Parents can log in to the CMISD Library Guardian Portal with the email address they have on file with their child's Skyward account. If you need additional support, you can access the setup instructions here or reach out to your child's campus.
